What are the doents I need to apply visa for Malaysia?

I could not get into the Malaysia Immigration official website because for some reason it is currently unavailable. I would like to inquire about the doents needed to apply visa/exemption order to Malaysia. I'm holding a Japan pport (diplomatic pport). Kindly fill me in about what form should I submit, doents, and photo needed. Thank you so much
